Community Innovations Team

Are you an older person who would like to get out and about more and have the opportunity to attend social activities or make new friends?

Do you have concerns about your health or wellbeing, or are you finding everyday activities increasingly difficult?

Then your community independence team could help?

What do the Community Innovations Team do?

We are here to help you maintain your independence, by providing you and your family with:

  • Support, advise and guidance;
  • assistance with linking you into appropriate health services and community activities;
  • specialist help if you are experiencing difficulties around the home, or with managing everyday activities, or if you are concerned about your health and wellbeing.

How does it work?

Our team include staff with knowledge and expertise in health, social care and voluntary services as well as support staff to assist you in accessing your local community if you need it.  You can contact the team yourself, so can your GP, or any of your family, friends or someone close.  We will talk things through and ask you what you want and need, and see if we can help.
